New digital form for records changes

The APVMA processes records change requests for access to the external portal, changes to company details and changes to contact details.

To improve efficiency for applicants, we have developed a single form which can be used for these tasks.

The form includes clear instructions regarding the roles and responsibilities of who should complete the sections, and is in a fillable PDF format which will allows it to be completed electronically and signed digitally. 

The new form is available now while older versions will be accepted to 2 August 2019. If you have any questions please contact: enquiries@apvma.gov.au.
